Introduction & Instructions

Purpose

This inventory is designed to help HPC coaches assess critical dimensions of your personal and organizational health:

The goal is to identify patterns, support growth, and guide meaningful coaching—not to evaluate or judge performance.

Confidentiality & Data Use

Your individual responses will be not be seen by anyone – including your assigned coach – unless you provide someone with your responses.
No one else in the organization will see your individual answers or scores.
Reports generated from this inventory will only include grouped data—aggregated by certain variables. No personally-identifying information (PII) will appear in any report.
The inventory platform and data storage are secured to protect all respondent information.

Your responses are not tied to your name or email in any system we store
Your answers will never be shared individually with department leaders or command staff
Only your coach will see your full responses—if you choose to share them
Aggregated results (by Shift, Company, Battalion) will help the organization improve systems and support, not evaluate individuals

Data Handling

HPC will store a de-identified version of your responses (no name, email, IP, or unit ID).
HPC may use the de-identified data to track group trends without compromising individual confidentiality.
Respondents will receive a copy of responses for personal use or coaching conversations.

Your data will remain confidential and only viewed individually by your coach.
